If you would like to determine your benefits, these are a few questions you can ask your insurance provider:

-
Does my health insurance plan include mental health benefits?
-
Does my plan cover psychotherapy and/or neuropsychological testing?
-
Does my plan pay for services provided by "out-of-network" providers?
-
Do I have a deductible? If so, what is it, and have I met it yet?
-
Does my plan limit the number of sessions I can have in a calendar year? If so, what is the limit?
-
Do I need written approval from my primary care physician for services to be covered?
-
What CPT Codes (related to mental health, wellness, and executive functioning) are approved for reimbursement?

**Full disclosure – Insurance companies require a formal diagnosis to be included on each superbill you submit for reimbursement. Additionally, your insurance company may ask for your therapy records. Since most of our clients desire the highest level of privacy, this is usually undesirable. Sometimes people go through arduous periods in their life and seek support but do not meet the criteria for a formal diagnosis. Furthermore, there may have been a time when an individual briefly met the criteria for a mental disorder, but generally does not. Diagnoses remain in your insurance records, even after the circumstances in your life have improved. Self-pay provides and allows the provision of services without the restrictions of insurance regulations, insurance adjusters, or their third-party adjusting agencies. Client confidentiality and discretion are of the utmost importance to us. Please let our Care Team know your insurance submission preferences.

CANCELLATION POLICY

At our practice, we understand that unexpected events can happen. However, in order to ensure that we can provide the best possible service to our clients, we ask that you please give us at least 48 hours notice if you need to cancel or reschedule your appointment. Thank you for your understanding and cooperation.

Last-minute cancellations and missed sessions may have an impact on other clients and our schedule. Therefore, clients will be charged $350.00 for an appointment that are not cancelled a minimum of 48 hours prior to the appointment time. Please email careteam@unfoldpsychology.com to cancel your appointment.

GOOD FAITH ESTIMATE

You have the right to receive a “Good Faith Estimate” explaining how much your care will cost. Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
-
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
-
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
-
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
-
Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.
